The goal of this grant program is to provide funding that enhances and supports grassroots work to prevent alcohol, tobacco, and marijuana use/misuse in BIPOC communities in Kentucky.

The Foundation for a Healthy Kentucky is partnering with the Kentucky Department of Public Health to administer a program to provide grants to support provider electronic health record (EHR) connections to the KYIR via the Kentucky Health Information Exchange (KHIE).

The Foundation for a Healthy Kentucky is looking to fund projects throughout the state that educate minoritized communities about adult vaccine recommendations and/or provide an opportunity for people to receive these immunizations.

This is a open application for unsolicited grant requests. All applications submitted here will be reviewed in the final quarter of the calendar year. We cannot promise that any grants will be made as it is dependent on what funds are available and what applications meet our priorities.
