Partnerships for Success Grants
Background
Partnerships for Success (PFS) provides funding to enhance and support Kentucky community-based organizations that serve communities with limited opportunities. Applicants should focus on reducing alcohol, nicotine (including vaping), and cannabis use/misuse and related consequences by implementing evidence-based prevention programs, practices, and policies that are trauma and resilience informed, and responsive to local and community context. The Foundation for a Healthy Kentucky (FHKY) is facilitating this funding opportunity as one of its pass-through grant programs. The project is supported by the Kentucky PFS 2023 Grant via the Substance Abuse and Mental Health Services Administration (SAMHSA) Grant H79SP083680.
Grants will provide up to $50,000 in funding for 12 months.
The grant period will end August 31, 2027. Applications are due by June 15, 2026.
Goal:
To provide funding that enhances and supports evidence-based strategies to prevent alcohol, nicotine (including vaping), and cannabis use/misuse in communities that are at higher risk in Kentucky.
Overview
This funding opportunity allows potential grantees to creatively address localized behavioral health-related issues. Funding is intended to support solutions that address the issues, identified by those who live and work locally, as most relevant to communities that are at higher risk in Kentucky.
Proposed projects should:
Focus on communities with limited opportunities as the primary focus of a proposed or existing project or program, including leadership and decision-making power.
Make communities that are at higher risk the primary beneficiaries of the project or program's outcomes.
Address a substance use challenge identified by the community.
Plan, develop, implement, and/or sustain a prevention initiative.
Grant size will vary based on the scope of the project. Eligible organizations may apply for grants up to $50,000.
Eligibility Criteria
Applicants Must:
Serve all members of their communities, including those that are at higher risk.
Provide services in at least one county in Kentucky.
Organizations that are currently receiving PFS grant funding may apply; however, preference will be given to new applicants.
Grantee Requirements
Depending on the project, awarded grantees may be expected to:
Participate in quarterly grantee meetings to share their work and learning with other grantees and Foundation for a Healthy Kentucky.
Submit quarterly progress reports.
Participate in evaluation and professional development activities for the grant and for the overall initiative.
Proposal Selection Criteria
The most competitive applications will:
Focus on communities with limited opportunities.
Authentically center communities that are at higher risk in the project, including through leadership and decision-making power.
Work to improve behavioral health outcomes by reducing risk factors and increasing protective factors in their communities.
Have goals for the grant period that are clear, feasible, and aligned with the goal of the PFS grant.
Demonstrate that the organization can achieve the goals outlined in the proposal with the budget, timeline, and staff capacity indicated.
Focus on a substance use (alcohol, cannabis, and/or nicotine) issue or challenge identified by Kentuckians who live and work as part of their community.
